Work Injuries (Workers’ Compensation). We also proudly assist individuals injured at work with an open claim. Prior Authorization by your insurance carrier’s claim adjuster/manager is required. A referral is usually necessary—download ours here or have your provider fax it to us from their internal referral system to 520.448.0819. We can help you with your prior authorization, please call us at 520-858-0350 with your claim information. Each employer in Arizona has an insurance carrier of their choice, or they may be self-insured, like Amazon, for example. Your employer should direct you to the correct entity for the handling of your medical claim. The Certificate of Insurance should also be posted on a wall in your workplace– you can look there too!
If you are a Federal Employee, work injuries are handled by the Department of Labor – Office of Workers’ Compensation Programs (OWCP). This includes Division of Energy Employees Occupational Illness Compensation (DEEOIC), Federal Black Lung Program (DCMWC) and Federal Employees Compensation Act (FECA) Program. All of our Licensed Massage Therapists are Providers with OWCP. OWCP Providers and can treat Federal Employees who have been injured while at work. Prior authorization is still required. Please call us so we can assist you and get set up in our system.
Everyday Aches and Pains, Sport & Hobby Injuries or Repetitive Strain Issues. We welcome a diverse range of patients who experience pain or discomfort or wish to improve some physical limitations. Many patients come seeking relaxation, stress relief, and a calmer nervous system. For non-insurance related issues, we do not require a referral to be seen in our office.
Referrals. If we are not billing your insurance, a prescription is not necessary for you to visit us. However, many of our referring providers will gladly write you a prescription so we can understand your exact diagnosis and assist with your medical expense documentation. As the landscape of insurance evolves, we highly recommend obtaining a Massage Referral regardless. Most primary care providers and specialists will be eager to provide one for you. This will be advantageous in cases where you wish to seek reimbursement from your health insurance provider.

Just ensure they include at least one diagnosis code (2-3 can be even more beneficial, if applicable), along with the recommended number and frequency of sessions—or simply state “at the therapist’s discretion”. Together, the therapist and patient set specific goals to achieve a reduction in pain, an increase in range of motion, and a return to daily activities and improved quality of life. The success of the treatment is determined by our collective ability—the patient and therapist—to meet these goals.
*Payments are made by the auto insurance company. The patient is ultimately responsible for any remaining balance not paid by the insurance company. If you do not have Medical Payments (MedPay) on your policy you have the option of paying out of pocket and seeking reimbursement yourself. We will always provide you with receipts and chart notes, upon request.
